1.
Non-Breeding: It is understood that if not
already done, this dog will be neutered or
spayed as soon as possible, in accordance with a
veterinarian’s age recommendation.
2. Transfer
of Ownership: This dog will not be
transferred to any other person by the adopter.
IF FOR ANY REASON
THE ADOPTION IS NOT SATISFACTORY, THE DOG WILL
BE RETURNED TO MID-FLORIDA RETRIEVER RESCUE,
INC.
3. Health:
The dog will be placed on an inoculation program
as prescribed by a licensed veterinarian in
order to assure and maintain the health of the
dog. The dog will receive a rabies vaccination
and be licensed in accordance with state and
local ordinances. The dog will be tested for
heartworm and heartworm preventative will be
administered as prescribed by a veterinarian.
4. Control: The
dog will not be allowed to run loose in unfenced
areas except in controlled sporting events or
dog parks. The dog will be under voice control
at all times.
5. Care:
Care and feeding instructions are part of this
agreement. The undersigned adopter accepts full
responsibility for maintaining a reasonable
level of care for this dog, including food,
water, grooming, sanitation, exercise, and
safety.
6. Housing:
it is understood and agreed that this dog has
been adopted as an indoor dog. Failure to keep
the dog living indoors with the family will
result in the removal of the dog from the home.
